Complete a medical history form. We need to collect this information not only for your health but also to gain an understanding of your previous dental experiences and treatment to ensure we can plan your treatment.

Meet with your dentist and discuss your individual needs, requirements and next steps. During your check-up and clean, the dentist will discuss any relevant treatment recommendations with you.

If further care is needed, your dentist will walk you through a personalised treatment plan and explain the options available.

Clinical indications may necessitate deferral of the offer pending completion of treatment diagnosed in the best interest of the patient, at the standard practice fee level – where this occurs, treatment that was included in the offer will be continued at a subsequent date when clinical indications are appropriate.
